Violations: Measuring cup used in bulk MSG with handle touching product. Frozen beef and shrimp noted thawing in room temperature water (corrected, product put under cold overflowing running water). Red poly cutting board badly scuffed with cut marks.
Plastic handles of knives used for food are noted with embedded soil. Handle areas of flip-top coolers and upright freezer dirty from frequent handling. Green wire racks (including walk-in cooler) dirty with greasy environmental and food soil. Some floor edges along baseboard areas dirty, primarily under equipment and racks.

Critical violations: Preparation cooler not operating properly to maintain potentially hazardous foods at or below 45 degrees (approximately 1 pound raw chicken, 1 pound raw beef, 1 pound raw shrimp, 1 pound cooked chicken, 2 pound cooked shrimp and 1 pound cooked pork between 48-52 degrees for less than 2 hours moved to walk-in cooler).
Violations: Wire shelving in walk-in cooler dirty. Some floor edgings under and around sinks dirty with heavy buildup.

Critical violations: Box of lemons and limes with mold and wilt found in walk-in cooler (corrected, entire box discarded). In walkin cooler, 12 inch deep Tub of onion soup (approximately 20 pounds, 12 inches deep) in walk-in cooler for less than two hours at 82 degrees on surface and 158 degrees in center; not an approved cooling method (correction, product was broken down into shallow pans and food safe bags of ice were added to speed cooling process).
Deep pan of curry paste (approximately 3 pounds) at130 degrees in glass door refrigerator for approximately an hour (corrected, pan was broken down into 2-inch-deep pans and returned to cooler).
Violations: Chilled foods found uncovered in walk-in cooler. Accurate thermometers not present in multiple coolers and refrigeration. All cutting boards found with deep staining and deep gouges and cut marks. Many shelves in kitchen are lined with tin foil, not a smooth and easily cleanable surfaces.
Shelving surfaces throughout facility are old and in disrepair. Most bulk storage have dirty exteriors with dried spills and residue. Several wet wiping cloths found on preparation surfaces, not stored in sanitizer solution between uses. Insides of flip-top preparation coolers, glass reach-in cooler, walk-in cooler and shelving all dirty with dried residue.
All cooking equipment fronts and sides dirty with dried food residue. Wall surfaces in kitchen dirty and have damage. Kitchen floor surfaces dirty, have tile damage at wall edges, especially under and behind cooking equipment.
